DESCRIPTION OF SERVICES: Medicare Certified Home Health and Hospice including Home Care Aide and Skilled Nursing Services. Public Health services including: immunizations, maternal child health, WIC, HOPES (in-home parenting), community education, FIP, EPSDT, Breast and Cervical Cancer program, Girl Talk, Senior Health physicals, case management for frail elderly, communicable disease follow-up, environmental health.

FEES: Charges based upon a sliding fee scale for self-pay clients. Alternate funding through Medicare, Medicaid, Elderbridge Agency on Aging, State and Federal grant monies, or private insurance.

ELIGIBILITY: Anyone residing in Hamilton County in need of services.

TRANSPORTATION: Limited

ACCESSIBILITY: ADA Compliant
